Disposition of Cross Card - This file is a two sided file and will need to be printed as such. Please print two copies on card stock. Keep one copy for yourself. The other is kept with your Chapter files. It is suggested that they remain in a file in the church office.
National Study Guide - Large file may require long download time.
Each Study Guide should be accompanied by one "Prayer Changes Me" and one "Rule of Life" brochure, ordered through the MJF Center. Small shipping fee for > 10 brochures.
Leaders Guide for National Study Guide - Includes the guidelines for leading a group in the Twelve Question Study Guide, Schedule for Eight Weekly Sessions and a class registration sheet
Junior Daugher Study Guide - This is the Study Guide for young women and girls seeking to become Junior Daughters. It is not mandatory that they complete the Guide to become a Daughter. It is simply a resource.

Transition Report Form - Use this form to report a change of name, address, transfer to new chapter, reinstatement, resignation or member's death. Please send form to dok1885@dok-national.org
Contribution Form - Please complete this form and send with any contributions to The Order. If a chapter is making a contribution, make sure the chapter's name appears in the "Given By" space. This insures that the chapter will be credited with the contribution.
